Vaya Health

Vaya Health JJBH initiative is a collaborative effort between the NC Department of Juvenile Justice and Delinquency Prevention and Vaya Health. The Multi-Disciplinary Team (MDT) makes service recommendations for youth in the target population across 32 counties.

Our Process
Screening

The Juvenile Court Counselor (JCC) administers the NC Risk Assessment (NCAR), NC Assessment of Needs (NCAN), and J-SOAP if applicable. For substance use, the JCC administers the CRAFFT. Referred youth receive a Comprehensive Clinical Assessment by an approved PIHP-enrolled provider.

Assessment

The JJBH multi-disciplinary team (MDT) includes Juvenile Justice (JCC), Vaya Health, behavioral health provider(s), family members, and other community partners. The MDT reviews the assessment, develops a care plan, and coordinates services.

Treatment

Treatment services include: Outpatient treatment, Intensive In-Home, Multisystemic Therapy (MST), Functional Family Therapy (FFT), Community Support Team, Facility-based Crisis, and Substance Abuse services.
